This article outlines the process for creating and submitting forms within Nucleus One. Administrators can create forms by signing in with admin privileges, navigating to the Forms section under Projects, and choosing to create a blank form, copy an existing one, or upload a PDF, then customizing it with various toolbox elements before sharing. Users can submit forms by locating the appropriate project and form, completing required fields (often marked with an asterisk), and clicking the Submit button, which triggers a workflow and results in a confirmation message.

Creating Forms

1. Sign into Nucleus One with an account with admin privileges.
2. Select Projects on the left sidebar. Select forms, and then the plus sign to add a form template.

3. Select one of the following options:
Create Blank: Creates a completely new form. You will have to name the form. 
Copy Existing: Copies an existing form which has already been created. The name of the form is copied from the copied form.
Uploading PDF: Allows you to upload a PDF, wherein you can drag and drop Nucleus One’s form elements. The name of the form is copied from the file name.


4. Customize the form as you wish. Click and drop elements from the toolbox.

Label, Image: Allows for the addition of non-question elements in the form.

Text Box: Allows for users to type in a custom response.

List Box: Allows a user to pick a response from a list.

Check Box: Multiple choice selection.

Radio Group: Single choice selection.

Sign and Date: Provides a signature and date from the user.

Upload: Allows for the uploading of files, such as PDFs and images.

Project Field: This links to configured fields in N1.




Click on preview to open the form, as it would be viewed by a user.





5. Select on the Share button, or share the link of the preview page.

Submitting Forms as a User


1. Go to the Projects section in Nucleus One.
2. Select the appropriate project where the form resides. Open the Forms Section.
3. Click on the Forms tab from the project menu, and choose the form which you need to complete.


4. Complete all required fields. Required fields are typically marked with an asterisk (*). You may also need to attach files, depending on the form design.
5. Click the Submit button once all data is entered correctly.
6. After submission, you’ll receive a confirmation message. The form is then routed according to the workflow process it’s tied to.
